We appreciate you may not be ready to talk about it yet and that recalling memories and events around the time that your loved one died can raise difficult emotions. Please remember this survey is completely voluntary, and it is very important you only complete this survey when you feel ready to do so. If you would prefer to respond at a later date, even if it's some weeks or months following the bereavement, that's perfectly alright.
