Overview.
It has been recommended that you have a central venous access
device (CVAD) inserted as part of the care that you are receiving.
The clinician inserting the device will explain the procedure and any
potential problems to you.
This passport aims to provide supporting information about the
procedure and about the care of your device to help ensure it is
managed safely.
At the back of the booklet there is a section for your health
professional who has accessed your line to complete. This is not to
replace any current documentation or policy that is in place. It is to
enhance communication, and continuity of care if you go to different
locations such as hospital, community or iv therapy unit and the line
is accessed.
Please remember to take it to your CVAD appointments or any
hospital admissions or when travelling.
