What is it?
• Transition and Preparing for Adulthood mean the same thing.
• You will be supported to manage your own health and wellbeing.
• We can make a plan together so you know more about adult health services.
When does Transition happen?
• Between the ages of 14 and 18.
• Conversations about transition start from the age of 14 but can depend on your needs.
Transition Plan
• At 14 you and your health team will make a plan.
• This plan will help you get ready for adult services.
• This is called a Transition Plan.
• We will look at this with you at least once a year.
Your Health
Your health team will tell you how to:
• Access therapy services.
• See a GP.
• Get an eye test.
• Get a check on your teeth.
• If you are 14 years old and have a learning disability you can go to the GP for an annual check-up.
• We will tell you how to do this.
Can we help you?
• Your health team will ask for your views and what you want to do when you are older.
• If there is anything you want to know please ask!
