Following your assessment with the Community Frailty service, you now have access to the local out of hours service which is provided by Fylde Coast Medical Services. This access will remain in place once you are discharged.
This is a local GP led service which operates outside of the opening hours of the Community Frailty Service and your GP Practice. The Community Frailty Service operates Monday to Friday during the hours of 09:00am to 17:00pm
The service can offer you advice and support with your health care needs out of hours and is offered across the Blackpool, Fylde and Wyre area as an alternative to calling the National 111 number.
A note has been added to your GP record to state that you now have access to the out of hours service. If you wish to see the information that is recorded on the out of hours service system, please ask your GP for a copy.
What is the out of hours service?
- The out of hours service is delivered by Fylde Coast Medical Service and is available on 0300 033 6520 between the hours of
- 6:30pm to 8:00am Monday to Friday
- 6:30pm Friday to 8:00am Monday (all weekend - including Bank holidays).
This is an alternative instead of calling the National 111 number.
- If you telephone the out of hours service, you will be asked a series of questions to enable the call handler to determine who you are, and the reason for your call.
- You will receive a telephone call back from a clinician if you have symptoms that require clinical advice or treatment – the aim is to call you back within 2 hours. The clinician will undertake a triage assessment and help determine the best health care for your needs.
- The Community Frailty Service and your GP will be notified that you have contacted the out of hours service the following working day.
- If you contact the out of hours service, you may be asked to provide some feedback about their services including what went well and what, if anything, they can do better. If you would prefer not to participate in this, please let them know.
Once you have been discharged from the Community Frailty Service, the out of hours service will remain available to you when your GP practice is closed.
