As a patient of our diabetes team, you are part of a comprehensive and supportive care program that is dedicated to helping you manage your diabetes and live a healthy, active life.
Our team is made up of experienced healthcare professionals, including endocrinologists, diabetes specialist nurses, registered dietitians, podiatrist, educators and admin colleagues who work together to provide personalised care and support.
Our in-patient diabetes team provides support and expert care to the patients admitted in our hospital. The inpatient multidisciplinary foot team visits patients weekly, whilst our doctors and DSNs (Diabetes Specialist Nurses) attend daily ward rounds and provide Ad-hoc patient education.
We offer a range of services to support patients with their diabetes management, including:
- Antenatal Consultant-led antenatal OPC (Tuesday AM)
- CSII/Pump Consultant-led OPC (Monday AM)
- Multidisciplinary Consultant-led Foot Clinic (Tuesday PM)
- Young Person & Transition Consultant-led clinics
- Type 1 Consultant-led clinic
- Advice and Guidance (For health care professionals in the community)
- Nurse led diabetes Helpline on weekdays from 9am -10.30am (only for patients under the care of the hospital diabetes team)
- General diabetes and Pump Nurse-led clinic daily in clinic room on ward C
- Structured education: DAFNE program
- Libre system starts and follow up
We celebrate various annual events throughout the year, including Insulin Safety Week, Diabetes Awareness Week, Hypo Week, and National Diabetes Awareness Day.
